Please bring with you: Medicare Card and Social Security Card; A print-out of medications from pharmacy or bottles; Proof of annual household income to determine whether you qualify for extra help; MUST HAVE driver's license or photo I.D.; Any letters from Social Security, Medicare or Medicaid which you may have received.
